DSCR I176: REQUIRED INSURANCE COVERAGE/COMPLIANCE

    In accordance with FAR Clause 52.228-5, Insurance - Work on a 
Government Installation (Section I), the following minimum amounts of 
coverage shall be maintained:

    WORKMAN'S COMPENSATION AND EMPLOYER'S LIABILITY - $100,000 
    (except in states with exclusive or monopolistic funds that 
    do not permit workers' compensation to be written by private 
    carriers or where maritime employment is involved, i.e., 
    Longshoremen's and Harbor Worker's Compensation Insurance is 
    required).  GENERAL LIABILITY - $500,000 per occurrence for 
    bodily injury.  AUTOMOBILE LIABILITY - $200,000 per person 
    and $500,000 per occurrence for bodily injury and $20,000 
    per occurrence for property damage.

        The Contractor shall sign below.

          COMPLIANCE WITH CONTRACT INSURANCE REQUIREMENTS

    The undersigned offeror acknowledges reading and understanding 
the insurance requirements specified in this solicitation, 
specifically, the requirements of clauses 52.228-5 and 52.228-9G01.  
The undersigned agrees to:

         (1)  Obtain and maintain the required insurance in at least 
the amounts and types specified in the solicitation during the 
performance period of any  resulting contract, including any 
extensions granted by contract modification;

         (2)  That each required insurance policy shall contain 
the endorsement specified in clause 52.228-5(b); and

         (3)  To furnish, at the Contracting Officer's request, 
current written proof of insurance for each subcontractor who shall 
perform work on the Government installation.  Such written proof 
must satisfy the insurance requirements of this solicitation and the 
resulting contract and shall be provided no later than five (5) 
days before each subcontractor commences work on the Government 
installation.

Further, the offeror acknowledges the Government will take appropriate 
remedial action, including the possibility of termination for default, 
if the offeror fails to obtain and maintain the required insurance 
coverages with the applicable endorsements for the prescribed periods.  
The offeror agrees this statement of compliance shall be a part of 
the contract and enforceable under any applicable provisions of the 
contract or as otherwise provided for by law.
